Returning Student Registration Guide
Before the start of each school year, parents/guardians of returning students are required to update their registration information with the school. This process will be done online.
Access to the returning student registration form is done through the PowerSchool Parent Portal. Be sure that you have each child linked to your parent portal. A returning student registration form must be completed for each student. You will have the option to copy common information onto each form.

Enrolling New Students
New students can be enrolled by scheduling an appointment. Please call the Superintendent’s Office at 419-395-1111 ext. 400. Please be prepared to bring in the documents listed below.
In compliance with the requirements established by Ohio law, the Ohio Revised Code and the Missing Children’s Act, along with the Ayersville Local Schools Board of Education, please bring the following information and documentation to your appointment.
The child’s Official Birth Certificate from state bureau of vital statistics
Photo identification (parent or guardian’s driver’s license/state ID)
Immunization records
Custody Document
If child is NOT living with BOTH biological parents listed on the official birth certificate, you must provide one of the following:
A copy of the complete court document (divorce decree, shared parenting agreement, etc.) including the cover page with filed/date stamp, page stating legal custodial/residential parent, and judge/magistrate’s signature page.
If legal custody has not been established, the petitioner has sixty (60) days from the time of enrollment to produce a signed Journal Entry of Judgment establishing a change in custody. In order for your child to be enrolled in Ayersville Local Schools during this waiting period, you must provide a copy of the filed/date stamped motion petitioning the court for a change in custody.
Proof of Residency - Parents must provide one of the following:
Current utility bills in your name (gas, electric, water)
Lease/rental agreement
If you are building a house in the district, a copy of the Builder’s agreement and a letter from Builder stating that occupancy will take place within 90 days from admission date; parent must agree to transport child to and from school until they occupy the new house.
